Chemical Use
Form
(for Chemical use in Animal
Rooms)
All investigators
working with animals using hazardous chemicals are required to fill
out a Chemical Use Form. The use of the hazardous chemicals
should be approved by the Chemical Safety Committee through the
Office of
Occupational and Environmental Safety.
