A biohazardous door posting form is required for any project involving the use of animals with zoonotic or recombinant DNA. However, the project must first be approved by the Inter-Institutional Biological Recombinant DNA Safety Committee (IBRDSC).  Once approval has been obtained, the door posting form must be signed by the Biological Safety Manager and the Chair of the Institutional Animal Care and Use Committee.

The Biological Safety Manager for Louisiana State University is Dr. Gregory Hayes. He is located in the Office of Occupational and Environmental Safety

Click here for a downloadable file which contains the door posting form in MS Word format.  The form can also be obtained from Dr. Hayes.
